Ventricular Assist Devices (VADs):

  • Mechanical pumps that support or replace the function of a failing heart
  • Types: LVAD (Left), RVAD (Right), BiVAD (Both)
  • Used as:
    1. Bridge-to-Transplant: Supports patients awaiting a donor hear
    2. Bridge-to-Recovery: Temporary support allowing heart function to recover
    3. Destination Therapy: Permanent support for patients who are not transplant candidates
    4. Benefits include reduced hospital stay, improved exercise tolerance, and better quality of life
    5. Implantable and wearable device options for patient mobility

Our ECMO and VAD programs are managed by trained specialists and supported by comprehensive critical care teams, ensuring round-the-clock monitoring, prompt intervention, and optimal outcomes.

ECMO (Extracorporeal Membrane Oxygenation):

  • Provides temporary support for heart and/or lung failure
  • Used in severe ARDS, cardiogenic shock, or during recovery from complex cardiac procedures
  • Acts as a bridge to recovery, transplant, or decision-making
  • Available in both VA (veno-arterial) and VV (veno-venous) modes
  • EEquipped with real-time monitoring and safety features for critical care

Liver Transplantation

Liver transplantation at Royal Care is a world-class, multidisciplinary program incorporating global surgical innovation and compassionate, personalized care.

Types of Transplants:

Living Donor Liver Transplantation (LDLT)

  • Uses a healthy segment from a living donor with advanced vascular techniques. Expertise in pediatric (including infants < 5kg ), obese, and complex cases with AI-driven graft selection.

Deceased Donor Liver Transplantation (DDLT)

  • Enhanced graft survival with Hypothermic Oxygenated Perfusion (HOPE). First in the region for zero-transfusion, domino, split, and re-transplants. Successful in high-risk and ABO-incompatible cases.

Split-Liver Transplantation

  • One donor, two lives advanced graft-splitting supports both adult and pediatric recipients.

Combined/Complex Transplantation

  • Dual organ (e.g., liver-kidney), re-transplants, and advanced cancer resections with curative intent.

Multidisciplinary Team

  • US & Japan-trained surgeons, expert hepatologists, transplant anesthesiologists, intensivists, dietitians, pharmacists, and dedicated coordinators, backed by advanced imaging, AI-driven logistics, and strict ethical standards.

Protocols

  • Rigorous pre-transplant workup, including fitness, nutrition, and psychological assessment.
  • Bridging therapies for liver cancer (TACE, TARE, systemic agents).
  • Seamless perioperative care with state-of-the-art anesthesia and post-operative ICU strategies.
  • Post-transplant, tailored immunosuppression and strict surveillance for graft function, infection, and long-term wellness.

Unique Achievements

  • Lowest vascular complication rates, pioneering DDLT perfusion, and 100% 5-year survival in elderly donor transplants - plus excellence in pediatric, high-risk, and sensitized cases.

Hepatectomy

Royal Care’s Hepatectomy service addresses both benign and malignant liver disease, employing sophisticated surgical approaches for both major and minor resections.

Indications & Approach

  • Treatment for liver tumors, trauma, or cysts with tailored, minimally invasive techniques and advanced pre-op planning.

Post-Operative Care

  • Dedicated liver ICU, rapid recovery protocols, advanced pain and nutrition management.

Special Capabilities

  • Expert in complex resections, vascular reconstructions, and combined multi-organ procedures.

Outcomes

  • Blood-conserving surgeries, shorter hospital stays, strong rehab, and integrated transplant-hepatectomy services within a full liver institute.

Pancreas Transplantation

A pancreas transplant restores natural insulin production for patients with uncontrolled diabetes, often combined with a kidney transplant (SPK). The donor pancreas is placed in the abdomen, connected to blood vessels and the intestine, while the native pancreas remains. With proper care and lifelong medication, it improves blood sugar control, quality of life, and long-term health.

Indications

  • For uncontrolled type 1 diabetes (with complications), end-stage renal disease with diabetes (SPK), and select cases of type 2 diabetes or chronic pancreatitis.

Donor & Recipient Prep

  • Pancreas is carefully retrieved, preserved, and quality-checked. Recipients undergo full evaluation; surgery is done under general anesthesia with the native pancreas usually left intact.

Surgical Procedure & Recovery

  • The donor pancreas is placed in the lower abdomen, with arteries and veins connected to the iliac vessels. Digestive drainage is linked to the small intestine, while the native pancreas is left intact. In SPK, the kidney is placed on the opposite side.
  • After surgery, patients recover in the ICU with close monitoring, immunosuppressive therapy, early mobilization, and dietary support. Most stay 6 - 10 days and learn lifelong graft and sugar management.

Benefits

  • Restores near-normal blood sugar control, eliminates or greatly reduces insulin dependence.
  • Reduces risk of long-term diabetes complications (kidney, nerve, eye, cardiovascular damage).
  • Dramatically improves quality of life and life expectancy for selected candidates.

Kidney Transplantation

Kidney transplantation is a life-saving procedure for patients with end-stage renal disease (ESRD) that replaces failed kidneys with healthy donor kidneys to restore normal kidney function.

Donor Selection and Evaluation

  • Thorough evaluation of living or deceased donors, including tests for compatibility, imaging, and health checks. Minimally invasive techniques are preferred for living donors.

Donor Kidney Retrieval

  • Living donors undergo laparoscopic/robotic surgery; deceased donor kidneys are preserved with cold storage or machine perfusion.

Recipient Preparation & Surgery

  • Recipients undergo pre-op evaluation and surgery under general anesthesia, with the donor kidney placed in the pelvis. Blood vessels are connected to iliac vessels, and the ureter is attached to the bladder.

Post-Operative Care and Recovery

  • Post-surgery, patients receive ICU monitoring, immunosuppressants, early mobilization, and nutrition support.
  • Hospital stay: 5 - 14 days with ongoing follow-ups.

Advanced Techniques and Innovations

  • Robotic surgery, machine perfusion, desensitization protocols, and paired donor exchanges improve safety, recovery, and outcomes.

Benefits and Outcomes

  • Restores renal function and eliminates or reduces the need for dialysis.
  • Improves quality of life and long-term survival.
  • Reduces complications associated with chronic kidney disease and dialysis dependency.
